The more specific the description, the more accurate the price and the fewer questions on the phone. For a boiler, the make and the code on the display. For a leak, where it is wet and whether the stopcock stopped it. For a drain, which fittings are affected. For a bathroom or a new heating system, the size of the room or the number of bedrooms and bathrooms, and what is there now. The map beside this text is the base town; engineers are routed by area, so the postcode in the form is what lets the visit be planned properly. What should I put in the form?

What is wrong, in your own words. The boiler make and the fault code if it is a boiler. The postcode and whether it is a house, a flat or a rented property. Anything about access, such as a porter or a tenant to contact. Photos are useful but not needed.

What happens after I send it?

An engineer reads it, calls to fill any gaps, and sends a written price or arranges a survey for installations. The visit is booked for a time that suits and that time is kept.

Can a tenant or an agent fill the form in for me?

Yes. Say who is paying and who should be contacted for access, and the paperwork goes to the right person.

I have an emergency right now. Is the form the right way?

Turn the water or the boiler off first. Then send the form with "emergency" in the description and what is happening; it is read as a priority. The emergency plumber page explains what to do in the first few minutes.

What do you do with my details?

They are used to price the job, arrange the visit and do the work, and for nothing else. They are not sold or passed to anyone who is not involved in the job.
